You will also be sent two html files. One is the form itself, the other is the page displayed when the form submission has been completed by the visitor.

Upload this attachment as a binary file to your cgi-bin directory, then set it's permissions to be executable

The security of this form requires you enter the domain name for which it will be used. This is a method used to prevent spammers from highjacking the form. The Email Address To is the address you want to receive the information entered when people use the form.
